I use Simple Practice software management for paperwork, video appointments and billing. Once you have an appointment scheduled, you will get an email from Simple Practice with a link to their secure client portal where you can fill out and submit all the forms sent with the email. If you are planning to use your health insurance, please be sure to upload a photo, front and back, of your insurance card.

If we are meeting via video for telehealth, you will also get an email reminder from Simple Practice approximately 24 hrs before each appointment with a link to log in to the video session.
